By participating in the microchip program, I acknowledge and agree to the following: The Fi Nano microchip used is ISO certified and ICAR registered, making it compatible with all international scanners. It operates on a 134.2 kHz frequency and is searchable through the AAHA 24/7 database. The microchip is preloaded in a sterile syringe with a small-gauge needle and individually wrapped to maintain sterility. I understand that I must register the microchip at https://nano.tryfi.com/register/chip?id=&petId= and that it is my responsibility to ensure all contact information remains accurate and up to date. I understand the microchip is not a GPS or tracking device and only works if my pet is scanned and my registration information is current. The microchip will be implanted under the pet’s skin, typically between the shoulder blades. Potential side effects may include mild discomfort or soreness, minor bleeding, temporary swelling or redness, hair loss, infection, or, in rare cases, microchip failure or tumor formation at the implantation site. I have been advised to consult a veterinarian before proceeding if I have any concerns. I acknowledge that The Infinity Project NFP is not a veterinary provider and assumes no responsibility for medical treatment or emergencies related to the procedure. Any concerns regarding the chip itself must be directed to the manufacturer. In the event of a change in ownership, it is my responsibility to update the registration. The Infinity Project NFP is not responsible for registration issues, transfers, or errors in the information I provide. I am solely responsible for restraining and controlling my pet during the procedure, and I release The Infinity Project NFP from liability for any injuries or incidents resulting from my pet’s behavior.
